TMS Therapy for Depression and Anxiety

Transcranial Magnetic Stimulation (TMS Therapy) was FDA-approved in 2008 as an alternative depression treatment that works. TMS works by using controlled magnetic pulses to awaken parts of the brain that are underactive in individuals with depression, anxiety, and OCD. TMS has one of the highest safety profiles for any depression treatment around today.

How TMS Works To Get You Better Fast

TMS uses targeted magnetic pulses to stimulate specific parts of the brain that are underactive in people who suffer from depression, anxiety, and OCD. During treatment, a TMS wand is placed gently on your scalp as the TMS machine delivers magnetic pulses to the brain. As these pulses are applied, TMS therapy helps reawaken brain activity in these parts of the brain, thus resulting in long-term symptom relief. Your care team will closely monitor your particular levels of depression and anxiety in order to best individualize the treatment for you.

The first step to starting TMS Therapy is an initial psychiatric evaluation from one of our psychiatrists in Santa Monica. Our flexible, easy scheduling allows for you to be evaluated virtually, in-person, before school, or even after work. If the psychiatrist or psychiatric nurse
practitioner determines that TMS is right for you, we will discuss the pros and cons of TMS therapy.Once you’ve decided to move forward, we will request approval from your insurance company. As a courtesy, our office will take care of 100% of the process so that you can focus
on feeling better. Once approved, you will come to the office approximately 5 days per week for about 6 weeks. After you complete a total of 30 sessions, you’ll come in for an additional 6 more sessions at a slower rate to complete your series. Many patients find that 36 sessions is
sufficient to notice remarkable changes in mood, anxiety levels, motivation, and energy. However, we have noticed that in certain cases, an additional 14-19 sessions of TMS can lead to even better results. Our team is experienced in obtaining insurance coverage for these
additional sessions if they are needed. For those who don’t wish to use their insurance or are looking for even more urgent relief, we offer Accelerated TMS. Accelerated TMS is a unique method of consenting an entire TMS series into only 5 days.

The Real Success Rate of TMS Therapy

TMS improves the symptoms of depression between 60-80% of the time, depending on the condition treated and protocol used. Research has found that multiple protocols–instead of just one–result in increased response rates to TMS and a higher likelihood for real remission. At
Bespoke Treatment, our independent research has found that we are able to at least markedly reduce particular symptoms 92.5% of the time. Our patients have described the feeling as if “the weight of an elephant has disappeared from [their] chest.” Many patients will remark that events that would otherwise be stressful just don’t feel as stressful anymore after doing TMS in our Santa Monica office. Almost all of our patients experience complete normalization of sleep and a newfound sense of energy.

Personalized TMS at Bespoke Treatment

Everyone has slightly different symptoms and physiology. We have mastered the art of fine- tuning individual TMS treatment protocols to increase effectiveness. Based on your results, we’ll tweak your individualized TMS therapy protocol every visit. That can mean changing the
duration of treatment, the frequency of stimulation, the number of sessions, and/or the placement on the head to target different brain areas. These parameters matter a lot when it comes to TMS, but yet they are ignored by most providers. There is no “one size fits all” approach to mental health treatment, and TMS is certainly no exception. Theta Burst Stimulation

In the past, a single TMS session took up to 37.5 minutes. With 5 weekly sessions, you’d be spending over 3 hours a week in treatment—not including parking. Theta Burst Stimulation (TBS) uses a patterned frequency which mimics the natural rhythms of the brain that occur
during neuroplasticity (aka learning). With TBS, your brain learns 10 times faster than with conventional TMS and your sessions only last 3 minutes.

fMRI Guided TMS

fMRI Guided TMS works by combining TMS treatment with functional neuroimaging (fMRI) and advanced brain mapping technology. The latest science suggests that when people do not respond to TMS, it is because an incorrect part of the brain was treated. With fMRI Guided TMS, your psychiatrist can accurately pinpoint specific dysfunctional areas of the brain involved in mood with surgical precision, ensuring that the right part of the brain will always be treated. The result is extraordinarily precise, truly individualized TMS placements for each patient.

Accelerated TMS

Accelerated TMS is an alternative schedule in lieu of the standard TMS course. This approach was most recently popularized at Stanford University, where researchers developed the SAINT™ protocol (Stanford Accelerated Intelligent Neuromodulation Therapy™), which involved
9-minute sessions, once per hour, for a total of 10 sessions per day for 5 consecutive days and led to an impressive 79% remission rate. Our Accelerated TMS program follows the same schedule of 5 days per week for 10 days, and is available with or without fMRI.

Medication-free treatments for depression and more

Transcranial Magnetic Stimulation (TMS) was first studied in humans in 1985 and received FDA approval for treatment-resistant depression in 2008. Initially, TMS was used for severe depression only, as an alternative to electroconvulsive therapy (ECT). However, over time, psychiatrists and researchers realized that TMS could be used earlier in the treatment process due to its safer side-effect profile compared to oral antidepressants. Japan even began offering TMS as an option before medication. Additionally, TMS has received approval in Europe as a
treatment not only for depression but also for PTSD, bipolar disorder, obsessive-compulsive disorder, chronic pain, Alzheimer’s Disease, multiple sclerosis, and Parkinson’s Disease.

TMS for Anxiety

Even though it is yet to be FDA Approved for Generalized Anxiety Disorder, TMS can be a profoundly helpful treatment for anxiety. In fact, many researchers believe that TMS could be a better treatment for anxiety compared to depression. It is not uncommon for our patients with
anxiety to experience a 50% or greater reduction in symptoms during their time with us. Our leadership team has published research regarding decreasing both anxiety and depression using TMS.

TMS for Post-Traumatic Stress Disorder (PTSD)

Multiple studies have found that TMS is helpful for reducing symptoms stemming from post-traumatic stress disorder (PTSD). TMS typically treats the prefrontal cortex, which is not usually considered to be associated with PTSD. However, the area of the prefrontal cortex that we treat is highly connected to a part of the brain responsible for our “fight or flight” response called the amygdala. By stimulating the right part of the prefrontal cortex, we are able to reach and regulate the amygdala. Our patients with PTSD will often remark about how much their sleep and anxiety have improved after beginning TMS.

TMS for Obsessive Compulsive Disorder (OCD)

In addition to depression, TMS is FDA-Approved for Obsessive-Compulsive Disorder (OCD). TMS can reduce the symptoms of OCD by stimulating a different part of the brain called the anterior cingulate cortex with a different pattern than what’s used for depression. If a patient is coming in for both depression and OCD, we will add-in a protocol for OCD at no cost. TMS is recommended for individuals with OCD who have not responded to medication and psychotherapy.

TMS for Bipolar Disorder

In addition to Major Depressive Disorder, TMS is FDA-Approved for bipolar depression. While TMS is not appropriate for those experiencing manic episodes, TMS can be quite helpful for those with bipolar depression. In fact, some researchers are of the belief that TMS is more effective for those with bipolar depression than unipolar depression, as the brain is “primed” for switching states.

Pros and Cons of TMS in Santa Monica

Before beginning treatment, patients may want to weigh the pros and cons of TMS therapy. The best parts about TMS are that it is safe, medication-free, has a high success rate, and produces results that last long after the treatment is over. Additionally, TMS can be covered up to 100% by most major insurers. The negative aspects of TMS are that it can cause mild headaches in some people and that patients must come into the office every day for 4 to 6 weeks, Monday through Friday.
