TMS Therapy for Depression and Anxiety

Transcranial Magnetic Stimulation (TMS Therapy) is an FDA-approved alternative depression treatment that works. TMS works by using magnetic pulses to stimulate regions of the brain involved in mood that are underactive in people with conditions such as depression, anxiety, and OCD. Unlike traditional antidepressant medication, TMS is a safe and effective treatment with minimal risk for long-term systemic side effects.

How TMS Works

The TMS device emits a magnetic field when placed upon a patient’s scalp. This magnetic stimulation activates brain cells underneath the scalp, which then trigger a complex process called neuroplasticity–the process of the brain forming new, healthier connections. As people continue to do more sessions of TMS, their brain returns to that of a “normal,” or otherwise non- depression brain. During the course of treatment, your particular levels of depression and anxiety will be closely monitored in order to continue to individualize the treatment for you.

A course of TMS therapy always starts with an initial psychiatric evaluation from one of our psychiatrists in Los Angeles. If the psychiatrist or psychiatric nurse practitioner determines that TMS is right for you, we will go through the pros and cons of TMS therapy with you carefully. After that, we typically request prior authorization from your insurance company. Once approved, you will come to the office 5 days per week for 6 weeks. After you complete 30 sessions, you’ll complete an additional 6 more sessions at a slower rate as you taper off of TMS. Many patients find that 36 sessions is sufficient to find relief from TMS, but we have noticed that an additional 14-19 sessions of TMS can lead to better results. Luckily, we are experts in obtaining insurance coverage for these additional sessions, if necessary. For those who don’t wish to use their insurance, we offer Accelerated TMS. Accelerated TMS is a novel way to condense an entire TMS course into just 5 days.

TMS Success Rate

Depending on the condition treated, TMS can generally improve the symptoms of depression between 60-80% of the time. At Bespoke Treatment, our independent research has found that we are able to at least markedly reduce particular symptoms 92.5% of the time. Our patients report that as they continue coming to treatment, the symptoms of depression start to just disappear. For many, they’ll start to notice that stressful life events simply won’t feel as stressful anymore. Others may notice sleep irregularities–such as insomnia, or the inability to fall asleep–become completely normalized and they are able to get a full nights’ rest again for the first time in years. And yet others are able to feel joy for the first time in years after a course of TMS therapy.

Theta Burst Stimulation

We highly recommend Theta Burst TMS. Older TMS techniques require 18-45 minutes of treatment. However, Theta Burst TMS is so advanced that only 3 minutes per session will be required. Theta Burst Stimulation works fast because it exactly mimics the natural rhythm of your brain. Since treatment time is so limited, you can come in on your lunch break for treatment before or after work or school.

fMRI Guided TMS

Wondering, “Does TMS actually work?” or you feel unsure about this treatment because you’ve read negative reviews, fMRI could be for you. The latest science suggests that when people do not respond to TMS, it is because an incorrect part of the brain was targeted. The left dorsolateral prefrontal cortex is always stimulated during TMS treatment. However, this part of the brain is quite large, and it is complicated to target precisely. Everyone’s brain anatomy is a little different. Moving just a few inches away can cause your TMS treatment to not work
correctly. By using fMRI to determine selective neuronal migration, our providers can confidently determine that we are stimulating the right area of the brain with neurosurgical precision.

Accelerated TMS

Studies over the last five years have shown that accelerated TMS protocols – primarily defined as 10 TMS sessions per day over just five days – can increase the chances of recovery up to 79%, as shown in the famous Stanford SAINT TMS study.

How TMS became one of the best treatments for depression

Transcranial Magnetic Stimulation was first studied in humans in 1985, and was first FDA- approved for treatment-resistant depression in 2008. In the beginning, TMS was used only for the most severe depression presentations. Patients considering electroconvulsive therapy
(ECT) were offered TMS as an alternative option for depression. As time went on, psychiatrists and researchers realized that there was no need to wait so long to start TMS, given that the side-effect profile is much safer than the typical first-line depression treatment:  oral antidepressants. Japan started offering TMS as an option before medication. It became approved by Europe’s regulatory (“CE Mark”) as a treatment not just for depression, but also for PTSD, bipolar disorder, obsessive-compulsive disorder, chronic pain, Alzheimer’s Disease, multiple sclerosis, and Parkinson’s Disease.

TMS for Anxiety

TMS can lessen anxiety in addition to treating psychiatric problems including depression and OCD, because anxiety symptoms are frequently involved in these conditions. TMS has also been shown to be beneficial for generalized anxiety disorders.

TMS for Post-Traumatic Stress Disorder (PTSD)

Multiple studies have found that TMS is beneficial for post-traumatic stress disorder (PTSD). TMS typically treats the prefrontal cortex, which TMS therapy can improve symptoms of obsessive-compulsive disorder (OCD). The FDA approved TMS for the treatment of OCD in 2018. Like TMS for depression, TMS is recommended for individuals with OCD who have not responded to medication and psychotherapy.
